Cadet Activity Consent Forms
These forms can be used for any cadet activity. They are in Microsoft Word format and can be filled in electronically by typing in the coloured entry fields, however they must then be printed, signed and taken to squadron/activity.
For any activity as a cadet both forms 1 and 3 must be completed. Any positive answer to a question on form 3 must then be clarified on Form 4.
Form 2 is for adult staff only.
